Community Partners is a program designed for parents in order to support their families in achieving goals and keeping their children safe and thriving in the home and community. This program is free of charge and is confidential. Community Partners provides direct assessment, home visitation and case management.
Our staff builds rapport with families and supports them in establishing one to three achievable goals. Based on the needs assessment and goals identified with the family, our staff links the family with appropriate community resources to achieve their goals. These resources include mental health services, homeless shelters, transitional housing, domestic violence support, health care organizations, hospitals, food pantries, workforce development, neighborhood and community action centers, schools and youth serving agencies.
Services are provided in La Porte, Porter, Jasper, Newton, Pulaski and Starke counties.
